To initiate a survey against your own facility within the RBA Online system you will go to the “Facility Info” page under the Company tab

You will be brought to the following screen that will hold a list of all of your facilities. From here, click on the [Deploy Survey] button:
 

This will open a window for you to select the Questionnaire you would like to deploy and one or multiple facilities to deploy to:

After making your selection, click the [Deploy] button to deploy the survey to the selected facility(ies):

When the facility administrator logs on to RBA Online, they will see a To Do that they have a survey to respond to.
