The Facility Risk SAQ utilizes a smaller question set and an updated scoring methodology. It also introduces an opportunity for scores to be improved by providing evidence of control mechanisms, and through an optional desktop validation of those controls.

Note: The score obtained on the Facility Risk SAQ may be different than the score obtained on the previous facility SAQ, as the updated scoring methodology was implemented to more accurately calculate risk. The two versions should not be considered equivalent. Further, what was formerly named the Facility Risk SAQ has been renamed simply as the Facility SAQ. The former Facility SAQ has been renamed the “Audit Prep Tool”, and it is no longer acceptable as a prerequisite to a VAP assessment, rather is available to assist facilities in preparation of the VAP.
